REGISTRATION FORMS 2024-2025

The pages of the Registration Form for either parish are below...

Guidelines are as follows:
Page 1 should be completed for 2 or less children.
Page 2 should be completed if there are more than 2 children.
Page 3 (class days, times, and tuition) should be completed by all.
Pages 4 through 8 are important informational pages.

Either mail the completed paperwork to the appropriate parish
or drop off the paperwork at the rectory
(paperwork can be left in Rectory mailbox if no one is
available to answer the door).
